Job Summary:

The MDS Coordinator is responsible for coordinating the Minimum Data Set (MDS) process for our facility. This role involves comprehensive assessment, care planning, and documentation to ensure compliance with state and federal regulations. As an MDS Coordinator, you will work closely with interdisciplinary teams to ensure that residents receive the highest standard of care and support.

Key Responsibilities:

• Conduct and Coordinate MDS Assessments: Ensure that all MDS assessments (Admission, Discharge, Quarterly, and Annual) are completed accurately and in a timely manner.
• Develop Care Plans: Collaborate with the clinical team to develop comprehensive care plans based on MDS assessments, ensuring they align with each resident’s needs and preferences.
• Regulatory Compliance: Ensure that the MDS process complies with all federal, state, and local regulations.
• Collaborate with Interdisciplinary Team: Work closely with nursing, therapy, social services, and other departments to ensure continuity of care and adherence to care plans.
• Resident and Family Advocacy: Ensure that resident and family concerns are addressed in a compassionate and professional manner.
• Data Management: Maintain accurate and complete MDS records, ensuring all required documentation is up-to-date and accessible.

Qualifications:

• Education: Bachelor’s degree in Nursing, Healthcare Administration, or related field preferred.
• Licensure/Certification: RN (Registered Nurse) license or LPN ( Licensed Practical Nurse). MDS certification preferred (e.g., MDS Coordinator Certification).
• Experience:
  • Minimum of 1-2 years of experience in a long-term care or skilled nursing facility setting.
  • Prior experience in MDS assessments and care planning is strongly preferred.
  • In-depth knowledge of MDS 3.0, RAI process, and current regulatory requirements is essential.
• Skills:
  • Strong organizational and time-management skills.
  • Ability to work independently and as part of an interdisciplinary team.
  • Excellent communication and interpersonal skills.
  • Detail-oriented with a commitment to accuracy.
