Job Description

Position: Occupational Therapist Full Time

Provides and directs Occupational Therapy services to patients including assessment, treatment, program planning and implementation, related documentation, and communication. Functions under physicians’ orders. Adheres to applicable principles and practices of occupational therapy, Facility policies and procedures and state regulations. Supervises Occupational Therapy Assistants and Aides

  • Required Qualifications:
  • Bachelor of Science in Occupational Therapy (Masters or Doctorate as required per graduation date of employee) from an accredited program.
  • Successful completion of the National Certification Examination for Registered Occupational Therapist.
  • License to practice occupational therapy within the state.

Major Duties and Responsibilities:

  • Optimize the physical abilities and skills of patients by performing or supervising therapy staff in the following activities: initiating referrals; evaluating and treating appropriate patients; complying with Minimum Data Set procedures; effectively participating in the care planning process; appropriately documenting progress toward identified objectives and communicating patient status and needs to the patient, the patient’s family, the staff and other professionals as appropriate.
  • Conducts screenings of patients at regular intervals to determine need for intervention/treatment.
  • Establishes individual patient schedules in conjunction with other team members.
  • Develops treatment plans by identifying a problem list, including long and short-term goals and methods to achieve identified goals.
  • Coordinates treatment plan with patient, family and other team members.
  • Implements and conducts treatment as outlined in the treatment plan.
  • Monitors patient’s response to intervention and modifies treatment as indicated to attain goals in accordance with physician’s orders.
  • Develops appropriate home or community programming to maintain and enhance the performance of the patient in his/her own environment.
  • Documents results of patient’s assessment, treatment, follow-up and termination of services.
  • Adapts treatment to meet the individual needs of the patient.
  • Provides/recommends/fabricates adaptive devices, orthotics or prosthetics. Trains patient, family and appropriate staff in the use or application of the above.
  • Participates in discharge planning.
  • Supervises students and volunteers.
  • Refers patients for treatment to Certified Occupational Therapy Assistants (COTA) by completing evaluation and initial plan of treatment and communicating necessary and sufficient information.
  • Supervises and directs COTA in the implementation of the treatment plan.
  • Supervises Rehabilitation Aides.
  • Reviews the quality and appropriateness of the total services delivered and of individual occupational therapy programs for effectiveness and efficiency, using predetermined criteria.
  • Participates in patient, family and staff education.
  • Confers formally and informally with other team members in coordinating the total rehabilitation program of the patient.
  • Adheres to Facility policies and procedures regarding documentation of occupational therapy services.
  • Attend in-service training and facility meetings as required.
  • Provides in-service education to the members of the patient care team and to members of the community.
  • Assumes responsibility for ongoing continuing education and professional development.
  • Submits billing, payroll, and expense documentation in an accurate and timely manner. Assists in the collection of statistics as directed.
  • Provides treatment in accordance with established standards of practice, department procedures and efficiency standards.
  • Maintains a safe and clean work area and adheres to facility/corporate safety standards.
  • Complies with all Infection Control, Universal Precautions, and OSHA standards for the healthcare professional.
  • Consistently demonstrates sound judgment in the evaluation, planning, implementation and follow-up of the patient therapy programs.
  • Maintains positive relationships and rapport with coworkers, patients, family members and facility personnel.
  • Reports to work on time and coordinates schedule to achieve maximum efficiency during assigned shift. Ability to work varying hours, including weekends and holidays.
  • Assists with and participates in Continuous Quality Improvement Program as required.
  • Adheres to established confidentiality standards as required by Facility concerning community and patient information.
  • Projects a positive and professional image at all times.
  • Performs other duties or special projects as assigned.
